In this short guide we will list the BG3 Barbarian class specific equipment. These items trigger on activation or deactivation of the Barbarian Rage ability, and typically heal the wearer or damage nearby enemies.

Reason’s Grasp
Reason’s Grasp are uncommon gloves that can heal the wearer (sort of).
When you deactivate your Rage or Frenzy early, you will gain 15 Temporary Hit Points. This is not true healing as it doesn’t restore your health pool, however it acts as a kind of damage shield that protects you from HP damage while it lasts.
15 Temporary Health is quite a lot, and since the Barbarian can rage several times per Long Rest you can get the buff pretty often. With that in mind, I strongly recommend getting this item if you have a Barbarian in your party.
You can find Reason’s Grasp in the Risen Road area of Act I. It is found inside a chest on the back wall of the cave that’s under attack by a pack of Gnolls.
Bonespike Helmet
The Bonespike Helmet is a very rare helmet that allows your Barbarian to deal AOE damage to enemies when they begin Raging, and also lets them make a melee attack that Frightens an enemy.
When the wearer activates their Rage or Frenzy, they deal 2d4 Psychic Damage to nearby (within 3m) enemies. Any targets who succeed a Wisdom Saving Throw will only take half of the damage.
In addition, you gain Menacing Attack, an action normally available to the Battle Master class that can Frighten the target, as well as dealing normal weapon damage to them. This variant of Menacing Attack ignores the Superiority Die mechanic that the Battle Master subclass uses.
As a final bonus, wearing this also gives an extra +2 to intimidation, which might come in handy if you’re frequently using your Barbarian for dialogue encounters.
To get the Bonespike Helmet, you must purchase it from Voiceless Penitent Bareki, who can be found in the Undercity Ruins of the Lower City, in Act III.
Bonespike Garb
The Bonespike Garb is a very rare Clothing item that has similar effects to Reason’s Grasp, but with more benefits added.
Just like with Reason’s Grasp, you’ll gain 15 Temporary Health with this item, however the Bonespike Garb triggers when you activate your Rage, rather than when you deactivate it. This means that with both of these items equipped, you can keep getting large amounts of Temporary Health by activating and deactivating your Rage, making your Barbarian much more durable.
The Bonespike Garb also reduces all incoming damage by 2, and deflects 3 Piercing Damage to enemies who hit you with a melee attack. All in all this is a really strong defensive item for Barbarians, and it has an excellent synergy with Reason’s Grasp.
The one caveat is that being a Clothing item, Bonespike Garb only gives you 10 base Armour Class, although it goes some way to make up for this with the aforementioned damage resistance. You can also improve your Armour Class by putting more points into Dexterity when using a Clothing item.
You can purchase Bonespike Garb very early in Act III. It is sold by Exxvikyap, the Dragonborn merchant in Rivington General. They work for the Blacksmith Gyldro Angleiron, which should help you to find the right location.
